Parkinson Disease – Symptoms Diagnosis and Prevalence

Parkinson’s Definition:

Parkinson's disease is a degenerative disorder of the central nervous system that often impairs the sufferer's motor skills and speech.

Parkinson's disease belongs to a group of conditions called movement disorders. It is characterized by muscle rigidity, tremor, a slowing of physical movement (bradykinesia) and, in extreme cases, a loss of physical movement (akinesia). The primary symptoms are the results of decreased stimulation of the motor cortex by the basal ganglia, normally caused by the insufficient formation and action of dopamine, which is produced in the dopaminergic neurons of the brain. Secondary symptoms may include high level cognitive dysfunction and subtle language problems. PD is both chronic and progressive.

Symptoms

Motor symptoms

The cardinal symptoms are:

  • tremor: normally 4-7 Hz tremor, maximal when the limb is at rest, and decreased with voluntary movement. It is typically unilateral at onset. This is the most apparent and well-known symptom, though an estimated 30% of patients have little perceptible tremor; these are classified as akinetic-rigid.
  • Rigidity: stiffness; increased muscle tone. In combination with a resting tremor, this produces a ratchety, "cogwheel" rigidity when the limb is passively moved.
  • bradykinesia/akinesia: respectively, slowness or absence of movement. Rapid, repetitive movements produce a dysrhythmic and decremental loss of amplitude. Also "dysdiadokinesia", which is the loss of ability to perform rapid alternating movements
  • postural instability: failure of postural reflexes, which leads to impaired balance and falls.
  • Gait and posture disturbances:
    • Shuffling: gait is characterized by short steps, with feet barely leaving the ground, producing an audible shuffling noise. Small obstacles tend to trip the patient
    • Decreased arm swing: a form of bradykinesia
    • Turning "en bloc": rather than the usual twisting of the neck and trunk and pivoting on the toes, PD patients keep their neck and trunk rigid, requiring multiple small steps to accomplish a turn.
    • Stooped, forward-flexed posture. In severe forms, the head and upper shoulders may be bent at a right angle relative to the trunk.
    • Festination: a combination of stooped posture, imbalance, and short steps. It leads to a gait that gets progressively faster and faster, often ending in a fall.
    • Gait freezing: "freezing" is another word for akinesia, the inability to move. Gait freezing is characterized by inability to move the feet, especially in tight, cluttered spaces or when initiating gait.
    • Dystonia (in about 20% of cases): abnormal, sustained, painful twisting muscle contractions, usually affecting the foot and ankle, characterized by toe flexion and foot inversion, interfering with gait. However, dystonia can be quite generalized, involving a majority of skeletal muscles; such episodes are acutely painful and completely disabling.
  • Speech and swallowing disturbances
    • Hypophonia: soft speech. Speech quality tends to be soft, hoarse, and monotonous. Some people with Parkinson's disease claim that their tongue is "heavy".
    • Festinating speech: excessively rapid, soft, poorly-intelligible speech.
    • Drooling: most likely caused by a weak, infrequent swallow and stooped posture.
    • Non-motor causes of speech/language disturbance in both expressive and receptive language: these include decreased verbal fluency and cognitive disturbance especially related to comprehension of emotional content of speech and of facial expression
    • Dysphagia: impaired ability to swallow. Can lead to aspiration, pneumonia.
  • Other motor symptoms:
    • fatigue (up to 50% of cases);
    • masked faces (a mask-like face also known as hypomimia), with infrequent blinking;
    • difficulty rolling in bed or rising from a seated position;
    • micrographia (small, cramped handwriting);
    • impaired fine motor dexterity and motor coordination;
    • impaired gross motor coordination;
    • Poverty of movement: overall loss of accessory movements, such as decreased arm swing when walking, as well as spontaneous movement.

Non-motor symptoms

Cognitive disturbances

  • slowed reaction time; both voluntary and involuntary motor responses are significantly slowed.
  • Executive dysfunction, characterized by difficulties in: differential allocation of attention, impulse control, set shifting, prioritizing, evaluating the salience of ambient data, interpreting social cues, and subjective time awareness. This complex is present to some degree in most Parkinson's patients; it may progress to:
  • dementia: a later development in approximately 20-40% of all patients, typically starting with slowing of thought and progressing to difficulties with abstract thought, memory, and behavioral regulation. Hallucinations, delusions and paranoia may develop.
  • short term memory loss; procedural memory is more impaired than declarative memory. Prompting elicits improved recall.
  • medication effects: some of the above cognitive disturbances are improved by dopaminergic medications, while others are actually worsened.

Sleep disturbances

  • Excessive daytime somnolence
  • Initial, intermediate, and terminal insomnia
  • Disturbances in REM sleep: disturbingly vivid dreams, and REM Sleep Disorder, characterized by acting out of dream content - can occur years prior to diagnosis

Sensation disturbances

  • impaired visual contrast sensitivity, spatial reasoning, colour discrimination, convergence insufficiency (characterized by double vision) and oculomotor control
  • dizziness and fainting; usually attributable orthostatic hypotension, a failure of the autonomous nervous system to adjust blood pressure in response to changes in body position
  • impaired proprioception (the awareness of bodily position in three-dimensional space)
  • reduction or loss of sense of smell (microsmia or anosmia) - can occur years prior to diagnosis,
  • pain: neuropathic, muscle, joints, and tendons, attributable to tension, dystonia, rigidity, joint stiffness, and injuries associated with attempts at accommodation

Autonomic disturbances

  • oily skin and seborrheic dermatitis
  • urinary incontinence, typically in later disease progression
  • nocturia (getting up in the night to pass urine) - up to 60% of cases
  • constipation and gastric dysmotility that is severe enough to endanger comfort and even health
  • altered sexual function: characterized by profound impairment of sexual arousal, behavior, orgasm, and drive is found in mid and late Parkinson disease. Current data addresses male sexual function almost exclusively
  • weight loss, which is significant over a period of ten years - 8% of body weight lost compared with 1% in a control group

Psychotherapeutic Interview for a Schizophrenic Patient

DO’S AND DON’TS FOR THE PSYCHIATRIC AND PSYCHOTHERAPEUTIC INTERVIEW:

There is no one right thing to say to schizophrenic patient. The most important job of the interviewer is to help to diminish the inner loneliness, chaos and terror that the schizophrenic patient is feeling. The challenge is to convey empathy without being regarded as being dangerously intrusive.

 

  • Don’t try to argue or rationally persuade the patient out of a delusion. This may lead to more assertion of delusional ideas.
  • Do listen. Listen to how patients experience the world. They may experience it as dangerous, bizarre , overwhelming and invasive. This may  be conveyed through their thought content. They may be angry, sad, afraid, or hopeless. They may feel that they have no privacy and no control over themselves. Try to understand what is their image of themselves.
  • DO acknowledge these feelings to the patient simply and clearly. For example if the patient respond that “When I walk into a room people can see inside my head and read my thoughts”. The clinician might respond to it as “ What are your feelings then” or “How do you feel then”.
  • Allow, the patient to speak. This helps the patient to feel that he is somebody important and has something important to say.
  • Be flexible about interview times. If the patient can tolerate only 10 minutes , tell him that the interview will resume later.
  • Be clear and reliable to the patient. This will help in maintaining trustworthiness.
  • Be straightforward with a patient. Do not pretend that a delusion is actually true, but convey that delusion is actually true for the patient. If  a paranoid schizophrenic says that “people are watching me all the time and they could know what I am thinking and see what I am doing” the doctor should say that “I can understand what you are feeling but I could not see anyone here who is keeping an eye on you”
  • DO pay attention, how the patient make you feel. Work over and analyze your feelings. Feel empathetic but do not get carried over by the feelings. If you feel annoyed find the reason for it.
  • Don’t  automatically laugh at a patient when something is said that seems funny. Laughing at a patient can convey disrespect and lack of understanding of the underlying terror and despair that many patient’s feel.
  • Do  respect a paranoid’s patient’s need for maintaining distance and control. Sometimes the paranoid patients are more comfortable when they are aloof as opposed to the expressions of warmth and empathy

Schizophrenia – Causes Symptoms and Treatment

 Schizophrenia is a brain disease that disables a person from everyday functioning. Understanding what schizophrenia is and the different characteristics behind it will often allow someone to help someone who is suffering from schizophrenia. Because this is a serious, mental illness, it is important to know what is involved in schizophrenia.

Schizophrenia is known as a brain disease that slowly begins to function one from normal functioning. From this disease, one begins to confuse realities and will begin to carry strange behaviors and personalities. From this, the person who develops schizophrenia is unable to function at a normal level.

Schizophrenia may be a result of several different factors. Several say that it is a result of genetics that run in the family. It is said that several genes that are combined together will cause schizophrenia in someone. Another reason why Schizophrenia may develop in someone is because of problems that they may have had at birth. Complications that stress out the fetus often result in schizophrenia developing later on. Another possible reason for schizophrenia is because of neural transmitters that move into the brain. Communications between nerve cells may cause the chemicals in the brain to shift and result in schizophrenia. These are only a few of several factors in which are being considered in relation to schizophrenia.

One of the early signs of schizophrenia is a constant state of confusion. This is then reinforced by sudden changes in behavior and personality. Several who are developing schizophrenia will often times do things that are out of their normal character. This will then lead to hallucinations and delusions that they believe occur in certain situations or at certain places. For instance, if one who is schizophrenic is in a certain situation, they will begin to believe that there are things or people around the area that aren’t really here. This can be done at several extremes.

As a result from this, they will begin to withdraw from friends and loved ones. This is not only because of the delusions, but also is a result of paranoia over what they believe these people can do. Often times, the delusions that are occurring are in relation to the person believing that they are a victim of a situation. Once schizophrenia worsens, it can produce problems with speech.

Disorganized thinking is another part of schizophrenia. Those with this disease will be known to jump from one subject to another with no connection. They will also not be able to put their thoughts together on one topic. This is a result of the hallucinations as well as the states of confusion that those with schizophrenia have developed.

Often times, those with schizophrenia will have uncontrollable feelings of fear and anger. This is often a result of the hallucinations that begin to occur. Often times, those with schizophrenia will feel like they are being victimized in several different situations. This includes the feeling that they are being persecuted or conspired against.

Those who have schizophrenia may respond in several different ways in order to control or minimize their thoughts. Several will turn to drug and alcohol abuse. Dependence on several external forces becomes dominant with a schizophrenic. You may also notice that the behaviors and personality shifts are not consistent in one who is suffering from this brain disease. Many times, suicide is one of the results from those suffering from schizophrenia.

Schizophrenia does not have a set treatment to it yet. Those that are experimenting with schizophrenia are still trying to find the exact cause of the disease as well as a balanced way to treat the symptoms. However, there are several different medications that can be used to reduce the symptoms that may be occurring. Before one decides to do this, it is important to look at the side effects, as well as the long term effects that may occur. Several will decide to go through rehabilitation or psychotherapy instead of being treated by a medication.

Schizophrenia is not a multiple personality disorder and is usually not dangerous to those around them. By understanding when someone is schizophrenic, and knowing what they are going through, you can help them and reassure their fears from the hallucinations and problems that they are having. By understanding the traits and behaviors of one that is schizophrenic, you can help them in adjusting to a somewhat normal life.

Tension headache cure with homoepathy

Tension headaches are very common, affecting up to 90% of the general population in city. Unfortunately, they're also among the most neglected and difficult types of headaches to treat. Add prunes to your diet to get key nutrients essential during pregnancy

Remember that the food you eat is the main source of energy for your baby growing inside. There is no special diet for a pregnant woman, but eating foods that provide all the nutrients to your baby to develop and grow is recommended. For the first 3 months, the diet of a pregnant woman should include foods rich in iron, folic acid and calcium. Prunes are of immense value during pregnancy for several reasons. Pregnant women usually experience constipation because the entire digestive system is influenced by hormonal changes during pregnancy. During pregnancy, the body produces more oestrogen than normal to ensure proper fetal growth; however, these hormones tend to slow down the intestinal movements leading to constipation and hard stools. One of these hormones, called progesterone, acts by relaxing muscle. The outside wall of the bowels is made up of muscle which relaxes and is, therefore, less able to move food and bodily waste along. Fiber is important because it pushes the intestines to work harder. It also absorbs liquid and ensures that the stool is soft. Prunes provide the critical amount of fiber required to help intestinal movement. Nausea (often accompanied by reflux) is common in the first trimester of pregnancy and may even carry forward to the advanced stages of pregnancy. Prunes can provide a tasty and nutritive snack while providing several essential nutrients in significant quantities. Iron is a key nutrient that is needed to develop a healthy baby. Prunes contain high levels of iron that help build hemoglobin in both the mother and baby, which not only prevents anemia but builds immunity, and combats fatigue. Prunes help maintain energy levels and calm the nerves. Iron requirements go up significantly during pregnancy (from 18 mgs to 27 mgs) and because it is usually not possible to get this from your diet, it is recommended that an iron supplement be taken. However, this can lead to gastro intestinal irritation and further aggravate the situation. Prunes and prune juice are a safe way of combating this issue. Potassium in prunes is known to reduce high blood pressure and act as a natural depressant as anxiety levels can be extremely high during pregnancy. Beta-carotene and Vitamin A is proven to promote eye development and healthy skin, boosting the immune system and assisting with blood production and prunes are known to be a good source of both. Prunes are a portable fat-free snack and can be easily had when hunger pangs strike during pregnancy.

Stressful environment is created by sharp edges continuously hitting the people all the time. To promote stress free atmosphere use music and plants.

To promote growth it is better to recruit young people, nuture their energy with water. Water may be moving water and a small water feature to generate energy. A carpet with blue and white blend colour is very harmonious. Quartz crystal clusters and objects help improving relationships. Six rod metallic wind-chimes are good to energize the areas further. Water symbolises wealth, but don't overdo it. Shelves should be avoided as they send out killing energy i.e. chi. Store the documents in cupboards not in open shelves. Kitchen and toilet should be small.

Select your office the back of which has a tall building. Water view is good but cranes are not. Well nurtured plants can do wonders as they grow and produce energy continuously. Generally, the offices must be brightly lit to promote adequate energy flow. Better still the natural light flowing in through glass. Avoid a glass front door. Left-hand side of your desk should always be higher than the right-hand side.

Place your table such that there is a wall behind you without a protruding column or overhead beam. Glazings are better in front or on the sides. The table should not face the door of your room or cabin. If you are sharing your cabin or room with your spouse or partner avoid siting on a single table or a joined table. The passage to your respective tables may be best from in between the two tables. Try these and start reaping fruits of success.

Vaastu & Feng Shui : Growth and Success

Feng Shui, the Chinese version is different in some respect as compared to vaastu, in the light of differences in climate, topography and local beliefs. India is a hot country mostly with a flat terrain, whereas China cold country with mountainous regions.

Yin & Yang (godly and demonic energies in vaastu), in Feng Shui, are basic components of universe and or responsible for creation and sustenance of all objects - living and non-living. Chi (pran in vastu) is the force produced due to the interaction of Yin and Yang. Feng Shui believes that with the manipulation of landscape we can get the best and balanced form of Chi. It also believes that all the four directions should have different levels to create excellent Chi.

Feng Shui ensures that Chi moves slowly and flow continuously towards all directions to enrich life in those desired happiness such as family harmony, growing career, good relationship, name, fame, children, etc.

Remedies in Feng Shui

Feng Shui believes that the necessary objects used by mankind, when used to enhance flow of Chi can be immensely helpful to fulfil the aspirations of the inmate in an abode or a workplace. There are 6 basic cures in the mode of Feng Shui Technology i.e. Mirrors, Plants, Fountains, Lights, Wind Chimes and Colours

1. Mirrors: Minors represent a doubling of revenues for the business. That is why Restaurants are also fond of using mirrors to symbolize a busier environment.
